Workshop 12: A Human Factors Approach to Accident Investigation and Prevention

Abstract
The human factor component of most accident investigation and reporting systems concentrates on pilot error. However, the term 'human factor' encompasses a multitude of other problems that directly impede safe performance in the cockpit. These additional factors can include inadequate supervision, organizational failure, dysfunctional psycho/social work environments, off-the-job stressors, maintenance error, and design flaws. An effective accident investigation and prevention program must account for these additional factors as well. This workshop provides safety professionals with the requisite knowledge and tools to conduct a thorough human error analysis of aviation accidents.
