Population Health Management: Building System Capital for Impact

Abstract
This session provides an overview of recent research on the implementation and impact of population health improvement strategies across the U.S. We examine key components of strategies designed for "whole population" improvement, as well as evidence about the health and economic effects of these strategies. Financing and governance models for these strategies are also considered.
