Promoting healthy equity through culturally-appropriate nutrition interventions to support Hispanic/Latine individuals with hypertension

Abstract
Objectives: Nutrition interventions for hypertension (HTN), such as the Dietary Approaches to Stop Hypertension (DASH) diet, fail to consider barriers to obtaining and utilizing fruits and vegetables (F/V). A paucity in the literature exists regarding cultural acceptability of nutrition interventions for Hispanic/Latine communities. This project aims to determine the effect improved access to a culturally-appropriate, medically-tailored "DASH box" has on Hispanic/Latine individuals with HTN via a community survey, individual interviews, and pilot trial. Methods: F/V preferences from 50 surveys supported the conceptualization of six DASH boxes. The boxes were displayed for 15 individual interviews gathering feedback on acceptability. Themes were assessed and finalized via consensus building. A 28-day clinical trial enrolling 20 participants collected pre- and post- measurements of blood pressure, weight, waist circumference, and skin carotenoids while receiving weekly DASH boxes.Results: Thematic analysis determined participants prefer fresh F/V, use staple items to compliment F/V, and experience barriers to accessing or using F/V. We found a significant improvement in systolic (mean difference of 3.4土7.3 mmHg, p<0.05) and diastolic blood pressure (3.0土5.6 mmHg, p<0.01) after the 28-day intervention.Conclusions: Attention should be paid to the voices of communities to develop culturally-appropriate nutrition interventions, meeting dietary recommendations for populations facing high rates of chronic diseases and food insecurity while managing chronic diseases. This work suggests culturally-appropriate DASH boxes may be an effective nutrition intervention to  lower blood pressure for Hispanic/Latine individuals with HTN.
