Abstract
Obstetric hemorrhage is the leading cause of maternal morbidity and mortality worldwide (Butwick & Goodnough, 2015).  It also represents an avoidable cause of maternal morbidity and mortality (Seacrist, Bingham, Scheich, & Byfield, 2018).  The most common treatment for obstetric hemorrhage is blood transfusion.  Members of the Jehovah’s Witness religious group do not accept blood transfusion as a medical treatment.  Therefore, women in this community are at a 44-fold increased risk of mortality related to childbirth compared to the general population (Singla, Lapinski, Berkowitz, & Saphier, 2001).  The California Maternal Quality Care Collaborative (CMQCC) has been instrumental in decreasing postpartum hemorrhage rates and reducing the related morbidity and mortality because of their development of an Obstetric Hemorrhage Toolkit (Main, et al., 2015).  The toolkit includes a comprehensive strategy to prepare for pregnant women of the Jehovah’s Witness faith with the use of a checklist which includes blood conservation strategies and possible alternatives to blood transfusion (Main, CMQCC Obstetric Hemorrhage Toolkit v. 2.0, 2015).  As part of an original plan for improvement, the checklist was modified and tailored to the needs of a Northern California Magnet® hospital using Kotter’s 8-step change model to implement a proactive approach in managing Jehovah’s Witness patients (Kotter, Leading change, 2012).  A qualitative analysis was utilized to measure changes in healthcare profession knowledge regarding attitudes toward the new strategy.  A business case model was also utilized to estimate cost savings.  Results yielded an increased awareness of strategies to improve outcomes for Jehovah’s Witness patients, improved cultural humility, and development of a new relationship with representatives of an underserved sub-population in the community.
